NEW PATIENT SERVICES

If you are a new patient, you can save time by printing the necessary forms out and filling them in before your appointment at Dr. Santangelo's office.

To utilize this feature:

(1) Make sure your printer is on

(2) Click on START (below) to load the "New Patient Forms"

(3) Once the forms are loaded, click PRINT
